Job summary

Family Home Health Network is seeking a licensed Physical Therapist to provide in-home PT services across the Chicagoland area. The role emphasizes assessment, treatment planning, and caregiver instruction to support patient independence.

The position requires IL licensure, CPR certification, and a valid IL driver’s license for travel to patient homes. Home Care Home Base experience is preferred, with benefits including health and retirement plans.

Qualifications

• Registered/licensed Physical Therapist in Illinois with current CPR.
• Minimum requirement for a physical therapy degree from an accredited program.
• Driver’s license and auto insurance required for patient home visits.

Responsibilities

• Provide physical therapy services to patients in home settings.
• Assess therapeutic/rehabilitative/functional status and contribute to care plans.
• Instruct patients, families and caregivers on therapy use and equipment.
