One Home Health LLC is seeking a Part-Time/PRN Physical Therapist to join our Chicago home health team. You will perform in-home PT evaluations and treatments, develop rehab plans, and monitor progress to support patient outcomes.

We value strong communication, accurate documentation, and the ability to work independently in a home-based setting while maintaining exceptional patient care and safety standards.

Qualifications

• Active Physical Therapist license in good standing (PT).
• DPT required or equivalent.
• Home health experience preferred but not required.
• Strong communication and documentation skills essential.
• Ability to work independently in a home-based setting.
• Professional, dependable, patient-focused approach.

Responsibilities

• Perform in-home physical therapy evaluations and treatments.
• Develop and implement individualized rehabilitation plans of care.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on exercises, mobility, and safety.
• Document visits accurately and efficiently in accordance with agency guidelines.
• Communicate patient progress with the interdisciplinary care team.
• Ensure compliance with clinical, documentation, and regulatory standards.
