Job summary

Family Home Health and Hospice is seeking a Physical Therapist to provide therapy services, assess functional status, and contribute to the total plan of care in a home health setting. You will guide patients and families, and address mobility, gait, and strength needs during visits across the Chicagoland area.

The role requires IL licensure, CPR, and driving with an insurance policy. Home Care Home Base experience is preferred.

Qualifications

• Baccalaureate or master's program in physical therapy approved by an accredited organization
• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the total plan of care.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
