Job summary

Family Home Health Network in the Chicagoland area is seeking a Physical Therapist to provide in-home physical therapy services, assess therapeutic status, and participate in developing the total plan of care. You will evaluate muscle strength, mobility, and gait, and instruct patients and caregivers in the use and care of therapeutic appliances.

Qualifications include a Baccalaureate or master's degree in physical therapy, current IL license, CPR card, and a valid IL driver's license with auto

Qualifications

• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services to patients.
• Assess and evaluate therapeutic/rehabilitative/functional status.
• Develop the total plan of care with the team.
• Assess muscle strength, mobility, and gait.
• Instruct patients, families and caregivers in use and care of therapeutic appliances and determine priority needs for physical therapy.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist


• Current CPR card


• Valid IL driver's license and auto insurance policy


• Home Care Home Base experience preferred
