Job summary

Family Home Health Network in Illinois is seeking a Physical Therapist to provide home-based PT services, assess functional status, and contribute to the total plan of care for patients in the Chicagoland area. You will evaluate strength, mobility, gait; instruct patients and families on therapeutic appliances; and help set priorities for therapy.

Excellent flexibility with own schedule and valid IL license required; CPR and driver's license/carry insurance preferred; Home Care Home Base

Qualifications

• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.
• Home Care Home Base experience preferred.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• Assess muscle strength, mobility, and gait.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist


• Current CPR card


• Valid IL driver's license and auto insurance policy


• Home Care Home Base experience preferred
