Job summary

Family Home Health Network in the Chicagoland area seeks a Physical Therapist to provide home health services, assess therapeutic status, contribute to the total plan of care, evaluate strength, mobility and gait, and educate patients and caregivers on therapy techniques.

Requirements include a bachelor’s or master’s degree in physical therapy, active Illinois license, CPR card, valid IL driver's license with auto insurance, and Home Care Home Base experience preferred.

Qualifications

• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.
• Home Care Home Base experience preferred.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the total plan of care.
• Assess for muscle strength, mobility, and gait.
• Instruct patients, families, and caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy and implement interventions.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist


• Current CPR card


• Valid IL driver's license and auto insurance policy


• Home Care Home Base experience preferred
