Job summary

Shore Pointe Care Center in New Jersey is seeking a dedicated Full-Time Physical Therapist Assistant to join our rehabilitation team in a skilled nursing facility.

You will assist with mobility, balance, and functional goals under a licensed PT, while collaborating with physicians, nurses, and therapy colleagues to deliver compassionate, patient-centered care.

Qualifications

• Active New Jersey Physical Therapist Assistant license or eligible to obtain.
• New grads welcome; mentorship and training provided.
• SNF/long-term care/geriatrics experience a plus.
• Strong communication and interpersonal skills.
• Ability to work in an interdisciplinary team.
• Compassionate, positive attitude focused on residents' well-being.

Responsibilities

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and functional mobility
• Treat residents with a variety of orthopedic, neurological, and post-surgical conditions under the supervision of a licensed Physical Therapist
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Report directly to the Physical Therapist and Director of Rehabilitation
