Job summary

Trinity Healthcare Talent Services, LLC seeks a dedicated Physical Therapist for a 400-bed facility. You will evaluate patients, develop individualized treatment plans, and collaborate with a multidisciplinary team to restore function and reduce pain.

Position offers 30-40 hours per week, Monday–Friday with no weekend rotation. Requires PT degree, valid state license, CPR certification, and 2+ years of healthcare experience in skilled nursing or assisted living settings.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy.
• Valid state license to practice as a Physical Therapist.
• Minimum of 2 years of experience in a healthcare setting (skilled nursing/assisted living preferred).
• CPR certification is required.

Responsibilities

• Conduct initial evaluations and assessments to determine needs.

Job Title: Physical Therapist

Salary Range: $40-$43 per hour

Hours: 30-40 hours per week, Monday through Friday, no weekend rotation

Facility: 400-bed facility, including a Long-Term Care unit with 40 beds, an Assisted Living unit with 100 beds, and an Independent Living Facility with 250-275 beds. The facility also has two licensed buildings, one with an Assisted Living unit and another with a combination of Assisted Living and Long-Term Care units. There is also an Assisted Living Dementia unit.

As a Physical Therapist, you will work closely with patients of all ages and conditions to develop and implement individualized treatment plans.

Key Responsibilities:

• Conduct initial evaluations and assessments to determine patients' physical therapy needs
• Develop and implement treatment plans based on patients' goals and needs
• Utilize a variety of techniques and modalities to help patients improve their physical function and reduce pain
• Monitor and document patients' progress and adjust treatment plans as necessary
• Educate patients and their families on exercises and techniques to continue their progress outside of therapy sessions
• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients
• Stay current with advancements and developments in the field of physical therapy
