Job summary

Vintage Faire Nursing & Rehab in Modesto, CA is seeking a dedicated Physical Therapist for a full-time in-house role. You’ll evaluate patients, design care plans, and deliver skilled therapy to geriatric clients in a supportive team environment.

PHYSICAL THERAPIST – FULL TIME

VINTAGE FAIRE NURSING & REHAB – MODESTO, CA

Join our IN-HOUSE Team in Modesto as a dedicated PT!

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Vintage Faire Nursing and Rehab. We offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Full Time: Flexible schedule with work-life balance

Pay: $68 - $72hour based on experience

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
