Job summary

Vintage Faire Nursing & Rehab is seeking a licensed Occupational Therapist for full-time in Modesto, CA. The in-house therapy team delivers geriatric rehabilitation with a patient-centered approach and adherence to regulatory standards.

Pay ranges from $68 to $72 per hour based on experience, with a $5,000 sign-on bonus and a comprehensive benefits package. Flexible scheduling is available to support work-life balance.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments to guide interventions.
• Provide rehabilitative and medically necessary treatments.
• Administer procedures as part of rehab plan (ADL retraining; exercises; modalities; devices).
• Document patient progress and adjust treatments in the clinical record.
• Communicate progress with supervisor and health team.
• Provide discharge summaries per regulatory requirements.

Join our IN-HOUSE Team in Modesto as a dedicated OT!

Our expanding network of in-house therapy Ensign-affiliate operatedhealthcare facilities is growing, and we are scouting for excellent therapists to join us at Vintage Faire Nursing and Rehab. We offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking Full Time Occupational Therapists for our in-house rehab program!

Compensation and Benefits

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.


• Utilizes standardized assessments and evidence-based practice to support clinical interventions.


• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.


• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.


• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.


• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.


• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.


• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.


• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
