Innovate Rehab & Wellness in St. Louis, MO seeks a Licensed Physical Therapist (PT) for a full-time role in a skilled nursing facility. You will evaluate patients, develop personalized treatment plans, and work with an interdisciplinary team to restore movement and quality of life.

The position offers an hourly pay based on experience, a Monday–Friday schedule with flexibility, and a supportive, patient-centered work environment focused on outcomes.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Valid state PT license required.
• Clinical experience is preferred but new grads are welcome.
• Strong communication, problem-solving, and patient-care skills are essential.
• CPR certification is preferred.

Responsibilities

• Evaluate patients and identify movement and functional limitations.
• Create and lead individualized treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Work closely with an interdisciplinary team for optimal recovery.
• Educate patients and caregivers on home programs and injury prevention.

Position Summary
