INNOVATE REHAB AND WELLNESS in Center, MO seeks a Licensed Physical Therapist (PT) for a full‑time role in a Skilled Nursing Facility. This position offers a competitive hourly rate based on experience and a collaborative clinical team.

You will evaluate patients, develop individualized treatment plans, and guide movement restoration through hands-on therapy, education, and progress tracking to ensure meaningful outcomes.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed; collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
