INNOVATE REHAB AND WELLNESS seeks a PRN Licensed Physical Therapist for a Skilled Nursing Facility in St. Elizabeth, MO. You will evaluate patients, develop personalized treatment plans, and deliver hands-on therapy to improve movement and daily function.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program)
• Valid state PT license
• Clinical experience preferred; new grads welcome

Responsibilities

• Perform comprehensive PT evaluations
• Develop personalized, evidence-based treatment plans
• Provide hands‑on therapy: manual techniques, therapeutic exercise, mobility training
• Educate patients and caregivers on home programs and injury prevention
• Track and document progress; adjust plans as needed
• Collaborate with physicians, nurses, OTs, and other clinicians
• Stay current with PT best practices through continuing education
• Ensure compliance with all regulatory and ethical standards
