Innovate Rehab & Wellness in Wellsville, MO is seeking a Licensed Physical Therapist to join our skilled nursing facility team on a full-time basis. • Valid state PT license.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track progress and adjust plans as needed.
• Collaborate with physicians, nurses, OTs and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with regulatory and ethical standards.

Position Summary

As a Physical Therapist, you will:
