Job summary

Angels Care Home Health is seeking a Physical Therapist to plan, implement, and evaluate patient care plans, restoring and maintaining patient well‑being. You will design therapy programs aligned with referrals and assessments, working collaboratively with an interdisciplinary team.

Qualified candidates hold a current Physical Therapist license, at least two years of experience, CPR and Hepatitis profile, and knowledge of durable medical equipment.

• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being.
• Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
