Job summary

Angels Care Home Health is seeking a licensed Physical Therapist to assess, plan, and implement patient-centered therapy programs in a home health setting. You will evaluate patient needs, develop treatment plans, and coordinate with an interdisciplinary team to restore or maintain function.

The role requires CPR certification, durable medical equipment knowledge, and at least two years of PT experience. A state license is required to practice; vehicle allowance and benefits are included for

• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
