Job summary

ReLive Physical Therapy in Illinois seeks a licensed Physical Therapist to deliver high-quality patient care and lead clinical teams. You will document evaluations, coordinate with physicians, and supervise assistants to ensure exceptional outcomes.

The role offers a full-time schedule (40 hours/week, Mon–Fri), competitive salary starting at $80,000 per year, and a comprehensive benefits package including medical, vision, 401(k), and continuing education opportunities.

Qualifications

• Graduated from an accredited Physical Therapist curriculum; MPT or DPT degree.
• Proof of licensure as a Physical Therapist in the state of Illinois.
• Current CPR/BLS Certification.
• Entrepreneurial spirit with energetic and team-player attitude.

Responsibilities

• Focused on providing superior clinical outcomes and rewarding experiences for our patients.
• Document all evaluations, treatments, and other pertinent patient interactions.
• Adhere to all HIPAA regulations regarding PHI.
• Adhere to Code of Ethics and Compliance program.
• Communicate, in a timely manner, with other professionals including physicians, case managers, etc. to provide status updates on patient’s plan of care and progress.
• Perform annual reviews of all staff members.
• Supervising Physical Therapist Assistants and Rehab Techs.
• Expectations for this role are 40 hours per week and 5 days per week in the clinic itself.

QUALIFICATIONS:

• Proven exceptional customer service and patient care experience
• Self-motivated with the willingness to exceed patient expectations
• Maintaining a positive attitude and build a fun, welcoming environment
• Graduated from an accredited Physical Therapist curriculum; MPT or DPT degree
• Proof of licensure as a Physical Therapist in the state of Illinois
• Current CPR/BLS Certification
• An entrepreneurial spirit with energetic and team-player attitude

Preferred Qualifications, but not required:

• 1+ years of outpatient PT leadership/supervision experience
• Sports Medicine experience
• Orthopedic experience
• Dry Needling certification
