1. Job Title and Status
  • Job Title: Physical Therapist
  • Employment Type: Part‑time, non‑exempt
  • Work Location: Revitalize Physical Therapy – Yorkville, Illinois (on‑site outpatient clinic)
2. Pay Range and Benefits
  • Compensation:
  • Anticipated pay range: $55.00 - $60.00 per hour, depending on experience and qualifications.
  • Sign‑on Bonus: $2,000
  • New hires are eligible to receive 50% of the sign‑on bonus after completing the 90‑day introductory period and the remaining balance after 6 months of continuous employment. Receipt of the full bonus is contingent upon completing 12 months of employment. If employment ends before 12 months of continuous employment, the full amount must be reimbursed.

3. Position Summary

Provide skilled physical therapy evaluation and treatment services to patients in an outpatient clinic setting. Develop and implement individualized treatment plans for orthopedic and musculoskeletal conditions, educate patients throughout their recovery, and maintain accurate clinical documentation in accordance with clinic and payer standards.

Deliver high‑quality, hands‑on care in a supportive team environment that values strong patient relationships, clinical collaboration, and work‑life balance.

4. Essential Job Duties

Patient care / clinical responsibilities:

  • Evaluate patients and develop individualized treatment plans based on clinical findings and patient goals.
  • Provide evidence‑based physical therapy interventions for orthopedic and musculoskeletal conditions.
  • Deliver hands‑on care and therapeutic exercise instruction in an outpatient setting.
  • Monitor patient progress and update treatment plans as appropriate.
  • Educate and motivate patients throughout their recovery journey.
  • Collaborate with other clinical team members to support continuity of care.
  • Maintain a safe, clean, and professional treatment environment.

Documentation / administrative responsibilities:

  • Complete accurate, timely documentation in accordance with clinic procedures and payer requirements.
  • Communicate effectively with patients, providers, and staff regarding treatment plans, scheduling needs, and follow‑up care.
  • Participate in staff meetings, training, and other clinic initiatives as needed.

General:

  • Follow clinic policies and procedures, including confidentiality and safety protocols.
  • Support a positive patient experience and a collaborative work environment.
  • Perform other related duties as assigned that are consistent with this role.
5. Physical / Functional Requirements

Physical ability to:

  • Stand and walk for extended periods during a typical shift.
  • Assist patients with therapeutic activities, transfers, gait, and positioning as appropriate.
  • Lift, move, or position equipment and supplies weighing up to approximately 50 pounds, with or without reasonable accommodation.
  • Frequently bend, reach, kneel, and move throughout treatment areas.
  • Use a computer, telephone, and other office equipment for extended periods.

6. Minimum Qualifications

Education and credentials:

  • Degree in Physical Therapy required; Master’s degree preferred.
  • Current Physical Therapy License in the state of Illinois required.

Experience:

  • Prior outpatient orthopedic or musculoskeletal experience preferred.
  • New graduates are welcome to apply; mentorship available.

7. Schedule

Part‑time: approximately 8 to 15 hours per week

Typical schedule options (This is flexible and can be discussed.):

  • Wednesday:12:00 p.m. – 6:00 p.m. OR 1:00 p.m. – 6:00/7:00 p.m. (flexible start time)
  • Friday:7:00 a.m. – 4:00 p.m. OR 8:00 a.m. – 5:00 p.m. OR 12:00 p.m. – 5:00 p.m.

We will also consider candidates available for one afternoon only on Wednesday or Friday.

No weekends. No holidays.

Patient volume emphasizes quality over quantity, typically 1 to 1.5 patients per hour, allowing ample time to work directly and closely with each patient.
