Continuum Therapy Partners, LLC is seeking a Home Health Physical Therapist Assistant in Eau Claire, WI and surrounding areas. This PRN role offers the freedom to create your own schedule and work with a supportive team that values clinicians.

We provide flexible daytime PRN positions with competitive pay and a focus on meaningful, one-on-one patient care in home health settings.

Formación

• Current Wisconsin license/certification required.
• Reliable transportation.
• Strong clinical and communication skills.

Responsabilidades

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• ?Associate Degree in Physical Therapist Assistant Program required
• ?Current, unencumbered state Physical Therapist Assistant License
• ?1 year of previous experience as a Physical Therapist Assistant preferred
• ?Implements individualized treatment plans under the supervision of a Physical Therapist
• ?Documents patient progress and reports changes in patient status to the supervising PT
• ?Educates patients on home exercise programs and proper use of assistive devices
