Job summary

PRN Home Health & Therapy is seeking a compassionate Physical Therapist Assistant (PTA) for a home health/SNF role in Fond du Lac and surrounding areas. The position offers 0.4 FTE with independent work and a supportive clinical team.

You will deliver PTA services, coordinate with the team for DME, and maintain proper treatment records while educating patients and caregivers on discharge planning. Wisconsin PTA license and CPR certification are required.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Wisconsin PTA license/certification.
• Current American Heart Association CPR certification.

Responsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist.
• Collaborate with the interdisciplinary team to identify and assist with obtaining patient-specific DME (mobility aids, wheelchairs, cushions, orthotics, etc.).
• Maintain accurate daily treatment records and progress notes as directed by the Physical Therapist.
• Educate patients and caregivers on safe discharge planning with the clinical team.

KeyResponsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist
