Job summary

Atrio Home Care is seeking a compassionate Physical Therapist to provide in-home services in Grand Rapids or Holland/Lakeshore areas. The role offers full-time or part-time options, with a $5,000 sign-on bonus and a balanced, patient-centered caseload within a 40-mile radius.

You will conduct evaluations, create treatment plans, collaborate with physicians, and educate clients and families while supporting discharge planning and mobility goals.

Qualifications

• Master's degree in Physical Therapy (required).
• Michigan PT license (required).
• 1+ year of PT experience (home health preferred).

Responsibilities

• Provide one-on-one in-home physical therapy services.
• Conduct evaluations and develop individualized treatment plans.
• Collaborate with physicians and interdisciplinary team members.
• Help clients regain mobility, strength, and independence.
• Document progress and support discharge planning.
• Educate clients, caregivers and families on therapy goals and techniques.

Physical Therapist | Home Visits – Grand Rapids or Holland| Full Time & Part Time

Schedule & Location:

• Now Offering: $5,000 Sign-on Bonus
• Days: Full & Part-time positions available
• Shift: Day shift (8 AM – 5 PM)
• Weekend Rotation: once every 4 weeks, no on-call
• Territory: Kent County
