Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to evaluate and treat residents across assisted living and independent living communities in the Grand Rapids area. You will develop and implement individualized treatment plans to improve mobility, reduce pain, and promote independence.

Qualifications

• Current State Physical Therapist (PT) license.
• CPR/BLS certification.
• Driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

About the Role
