Atrio Home Care in Holland/Lakeshore is seeking a compassionate Physical Therapist to provide in-home services within a 40 mile radius. Flexible full- and part-time opportunities offer autonomy, supportive teamwork, and a meaningful schedule.

You will evaluate clients, design personalized treatment plans, and collaborate with physicians and the care team to improve mobility and independence. A sign-on bonus and mileage reimbursement enhance this role.

Qualifications

• Master's degree in Physical Therapy (required).
• Michigan PT license (required).
• 1+ year of PT experience (home health preferred).
• Geriatric experience highly valued.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Provide one-on-one in-home physical therapy services.
• Conduct evaluations and develop individualized treatment plans.
• Collaborate with physicians and interdisciplinary team members.
• Help clients regain mobility, strength, and independence.
• Document progress and support discharge planning.
• Educate clients, caregivers and families on therapy goals and techniques.

• Shift: Day shift (8 AM – 5 PM)
