Qualifications

• Master's degree in Physical Therapy is required.
• Active Michigan PT license is required.
• At least 1 year of PT experience; home health preferred.
• Geriatric experience is highly valued.

Responsibilities

• Provide one-on-one in-home physical therapy services.
• Conduct evaluations and develop individualized treatment plans.
• Collaborate with physicians and interdisciplinary team members.
• Help clients regain mobility, strength, and independence.
• Document progress and support discharge planning.
• Educate clients, caregivers and families on goals and techniques.
