Synergy Care, Inc. is hiring a full-time Physical Therapist for an outpatient clinic in Fort Worth, TX. The role focuses on evaluating patients, developing individualized treatment plans, and advancing discharge planning.

You will supervise assistants, participate in case management, and deliver high-quality therapy in accordance with state regulations and company guidelines. NBCOT certification and current Texas PT license are required.

Qualifications

• Graduate of an accredited Physical Therapy program (or graduating within 6 months)
• NBCOT certification required
• Current Texas PT license required
