Synergy Care, INC is hiring a full-time Occupational Therapist for an outpatient clinic serving an AL/IL/Memory Care community in Fort Worth, TX. The role focuses on delivering high-quality OT services and developing individualized treatment plans in a collaborative, patient-centered environment.

The ideal candidate will have an accredited OT degree, NBCOT certification, and active or eligible Texas licensure, with a commitment to teamwork and quality patient care in an outpatient setting.

Qualifications

• Graduate of an accredited Occupational Therapy program (or graduating within 6 months).
• NBCOT certification required.
• Current or eligible state OT license required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide ongoing treatment and discharge planning.
• Supervise assistants and aides.
• Participate in case management.
• Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines.
