Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our team in Fort Wayne, IN. You will evaluate residents, develop treatment plans, and deliver skilled PT services in assisted living and independent living settings. Responsibilities include supervising PTAs, documenting in EMR, and educating residents and caregivers.

Indiana PT licensure and CPR/BLS are required; driver’s license and reliable transportation preferred. Part-time scheduling with potential weekend options.

Qualifications

• Active PT license in Indiana.
• CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Experience in geriatrics/home health/assisted living preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including exercise, gait and balance training.
• Reassess progress and modify plans as needed.
• Supervise PTAs in accordance with state regulations.
• Document care in EMR in a timely and accurate manner.
• Educate residents and staff on treatment plans and home exercises.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
