Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Newark, OH. You will evaluate, develop, and implement individualized treatment plans to improve mobility, reduce pain, and maintain independence for residents in assisted living and independent living communities.

You will supervise PTAs, document in the EMR, and educate residents and caregivers on treatment plans and home exercise programs.

Formación

• Active Physical Therapist license in the state
• Current CPR/BLS certification
• Valid driver’s license and reliable transportation
• Experience in geriatrics/home health/assisted living or skilled nursing preferred but not required

Responsabilidades

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait, balance, transfer, and functional mobility interventions.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Document using the EMR in a timely and accurate manner.
• Educate residents, caregivers and staff on plans and home exercise programs, falls prevention and safe mobility techniques.
• Collaborate with interdisciplinary team for coordinated patient-centered care.
• Maintain a productive clinical caseload while delivering meaningful therapy services.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

Evaluate residents and develop individualized plans of care.

Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.

Reassess patient progress and modify treatment plans as appropriate.

Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.

Complete timely and accurate documentation using the electronic medical record (EMR).

Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.

Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

Current State Physical Therapist (PT) License

Current CPR/BLS Certification

Valid driver's license, reliable transportation, and auto insurance

Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
