Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing Michigan team. You will evaluate residents, develop treatment plans, and provide skilled PT services in assisted living and independent living communities.

The role emphasizes mobility, pain reduction, function restoration, and collaboration with an interdisciplinary team. Per-visit or salary compensation is based on experience, with flexible scheduling and a therapist-owned culture.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

About the Role
