Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Central Ohio. You will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and maintain independence for residents in assisted and independent living communities.

You will collaborate with an interdisciplinary team, supervise PTAs, and document care using the EMR system.

Qualifications

• Must hold a current State PT license.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Experience in geriatrics/home health or assisted living preferred but not required.

Responsabilités

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait and balance training.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation in EMR.
• Educate residents and caregivers on treatment plans and home programs.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload while delivering high-quality therapy.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
