Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing Ohio team in Powell. You will evaluate and implement individualized plans to improve mobility, reduce pain, and restore independence for residents in assisted living and independent living communities.

As a therapist-owned organization, we emphasize mentorship, collaborative care, and career growth as we expand across Ohio.

Qualifications

• Current State Physical Therapist license.
• Current CPR/BLS Certification.
• Valid driver's license and reliable transportation.
• Geriatrics/home health/assisted living experience preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait and balance training, transfers, and mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) per state regulations.
• Complete timely and accurate EMR documentation.
• Educate residents, caregivers, and staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members for coordinated, patient-centered care.
• Maintain a productive clinical caseload and deliver exceptional therapy services.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
