Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing Southeast Michigan team in assisted living and independent living communities. You will evaluate residents, develop individualized treatment plans, and implement interventions to improve mobility, reduce pain, and maintain independence.

You’ll collaborate with an interdisciplinary team, mentor PTAs, document in EMR, and help enhance quality of life for older adults.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
