All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope.

Position

Field Physical Therapist

Responsibilities

• Provides comprehensive, skilled physical therapy services/interventions in accordance with physician orders.
• Comfortable with start of care (SOC), Evaluations, Resumption of Care (ROC), Recertification (Recert), and Discharges (DC).
• Assesses patient skilled needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with the interdisciplinary team to plan, evaluate team\'s goals for each individual patient.
• Educates, instructs, and trains patient, family, responsible caregiver on patient\'s physical therapy program.

Requirements

• Active/Registered by the Physical Therapy Board of California.
• Graduate from a Physical Therapy accredited school.
