Job summary

Qualifications

Job Description

We are seeking a skilled and compassionate Physical Therapist (P-Physical Therapist) to join our team at a skilled nursing facility/assisted living facility (SNF/ALF). As a P-Physical Therapist, you will work with a diverse group of patients to help them regain their physical strength, mobility, and independence.

Key Responsibilities

• Conduct initial evaluations and develop individualized treatment plans for patients based on their specific needs and goals.
• Implement and monitor treatment plans, adjusting as necessary to ensure progress and optimal outcomes.
• Use a variety of therapeutic techniques, exercises, and equipment to help patients improve their strength, flexibility, balance, and coordination.
• Educate patients and their caregivers on proper techniques for performing exercises and activities to promote long-term wellness and prevent future injuries.
• Collaborate with other healthcare professionals, such as occupational therapists and physicians, to provide comprehensive care for patients.
• Maintain accurate and timely documentation of patient progress, treatment plans, and other important information.
• Stay updated on the latest advancements and best practices in physical therapy to continuously improve patient care.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• Minimum of 1-2 years of experience working as a Physical Therapist, preferably in a SNF or ALF setting.
• Strong knowledge of anatomy, physiology, and kinesiology.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
