Job summary

WelbeHealth seeks a Physical Therapist to join a patient-centered interdisciplinary team and deliver restorative and rehabilitative PT services. Focus on care planning, safety, and independence for participants in our IDT-driven model.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• Valid California Physical Therapy License, or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job description

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On The Day-to-day, You Will
