WelbeHealth is seeking a Physical Therapist to join the interdisciplinary team and deliver restorative and rehabilitative PT services. The role focuses on care planning, independence, and safety, with care delivered in facility and home visits as needed.
Responsibilities include assessments, care planning, and documentation, plus supervising assistants. Requires CA PT license and CPR/First Aid within 180 days; base salary plus bonus and strong benefits package.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.
