Job summary

WelbeHealth is seeking a Physical Therapist to join the interdisciplinary team and deliver restorative and rehabilitative PT services. The role focuses on care planning, independence, and safety, with care delivered in facility and home visits as needed.

Responsibilities include assessments, care planning, and documentation, plus supervising assistants. Requires CA PT license and CPR/First Aid within 180 days; base salary plus bonus and strong benefits package.

Qualifications

• Bachelor’s degree in physical therapy (required).
• Master’s or doctoral degree preferred.
• Valid California Physical Therapy License or proof of license eligibility; CPR and First Aid certification within 180 days of hire.

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, gait and ADL training, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants and provide training to implement Functional Maintenance Programs (FMP).
• Provide training to staff, participants, and caregivers on use and care of therapeutic equipment.

Education

Job description

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On the day-to-day, you will:

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals

• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job requirements include:

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
