Job summary

WelbeHealth is seeking a Physical Therapist to join the interdisciplinary team and lead restorative therapy services for participants. You will plan and coordinate care, deliver interventions, and collaborate with therapists and providers to maximize independence and safety.

Ideal candidates have outpatient, acute, inpatient, or skilled nursing experience and hold a California PT license. Compensation includes base pay plus a bonus within a comprehensive benefits package.

Qualifications

• Bachelor's degree in physical therapy.
• Valid California Physical Therapy License or license eligibility.
• CPR and First Aid certification within 180 days of hire; preferred at hire.

Responsibilities

• Conduct PT assessments in our facility and, occasionally, in the home.
• Participate in IDT meetings to develop care plans.
• Deliver and document interventions as per plans.
• Provide active/passive exercise, gait and ADL training; modalities as appropriate.
• Supervise Therapy Assistants and train staff on maintenance programs.
• Educate participants, families and staff on therapeutic equipment use.

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On The Day-to-day, You Will

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job Requirements Include

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
