Emerald Nursing & Rehab Columbus is seeking a compassionate Physical Therapist (PT) to join our rehab team in Columbus, NE. This PRN role focuses on resident-centered care, evaluating residents and delivering therapy to improve mobility and independence.

You''ll collaborate with nurses and physicians, educate residents and families, and document care in accordance with regulatory standards, contributing to exceptional outcomes in a supportive team environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license in good standing.
• Experience in skilled nursing or rehab is preferred.
• Excellent communication and teamwork skills.

Responsibilities

• Conduct comprehensive evaluations and create personalized treatment plans tailored to each resident's goals.
• Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function.
• Monitor progress, celebrate achievements, and adjust treatment plans to maximize results.
• Collaborate closely with nursing staff, physicians, and interdisciplinary team members to provide holistic, high-quality care.
• Educate residents and families on treatment strategies, recovery expectations, and wellness goals.
• Maintain timely and accurate clinical documentation in accordance with regulatory standards.
