Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license in good standing.
• Experience in skilled nursing or rehab is preferred.
• Excellent communication and teamwork skills.

Responsibilities

• Conduct comprehensive evaluations and create personalized treatment plans tailored to each resident's goals.
• Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function.
• Monitor progress, celebrate achievements, and adjust treatment plans to maximize results.
• Collaborate closely with nursing staff, physicians, and interdisciplinary team members to provide holistic, high-quality care.
• Educate residents and families on treatment strategies, recovery expectations, and wellness goals.
• Maintain timely and accurate clinical documentation in accordance with regulatory standards.
