Job summary

Emerald Nursing & Rehab Columbus is seeking a passionate Physical Therapist (PT) to join our resident-centered rehab team in Columbus, NE. This PRN position focuses on evaluations, personalized plans, and collaborative care to maximize residents’ independence.

You will work with nursing and medical staff to deliver evidence-based interventions, educate residents and families, and maintain compliant documentation. This role emphasizes compassionate, high-quality outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in good standing.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Excellent communication, interpersonal, and problem-solving skills.
• Compassionate, resident-focused mindset and a passion for improving quality of life.
• Ability to work effectively in a collaborative, team-oriented environment.

Responsibilities

• Conduct comprehensive evaluations and create personalized treatment plans tailored to each resident’s goals.
• Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function.
• Monitor progress, celebrate achievements, and adjust treatment plans to maximize results.
• Collaborate closely with nursing staff, physicians, and interdisciplinary team members to provide holistic, high-quality care.
• Educate residents and families on treatment strategies, recovery expectations, and wellness goals.
• Maintain timely and accurate clinical documentation in accordance with regulatory standards.
