Job summary

We are seeking a highly skilled Physical Therapist to join our team at an Assisted Living Facility, Independent Living, and Skilled Nursing Facility at the same location. You will evaluate and treat patients to improve mobility and quality of life, working within a multidisciplinary team.

The role requires a state license, a degree in Physical Therapy, and at least 2 years of clinical experience, with emphasis on senior living settings.

Qualifications

• Requires a PT degree and state license.
• Minimum 2 years of clinical experience.

Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations.
• Develop individualized treatment plans based on evaluation findings and patient goals.
• Administer therapeutic exercises and interventions to improve strength, balance, coordination, and range of motion.
• Utilize modalities such as heat, cold, ultrasound, and electrical stimulation.
• Educate patients and families on proper techniques and exercises to continue at home.
• Monitor and document patient progress and adjust treatment plans as needed.
• Collaborate with other healthcare professionals for coordinated care.
• Maintain accurate and up-to-date patient records and adhere to regulations.

Salary Range: $45,000 - $48,000 per year

Job Type: Full-time, 40 hours per week, Monday-Friday with occasional weekend shifts as needed

Location: Assisted Living Facility (ALF), Independent Living (IL), and Skilled Nursing Facility (SNF) at the same location

As a Physical Therapist, you will be responsible for evaluating and treating patients with physical disabilities or injuries to improve their overall mobility and quality of life.

Key Responsibilities:

• Conduct comprehensive evaluations of patients' physical abilities and limitations
• Develop individualized treatment plans based on evaluation findings and patient goals
• Administer therapeutic exercises and interventions to improve strength, balance, coordination, and range of motion
• Utilize modalities such as heat, cold, ultrasound, and electrical stimulation to relieve pain and promote healing
• Educate patients and their families on proper techniques and exercises to continue at home
• Monitor and document patient progress and adjust treatment plans as needed
• Collaborate with other healthcare professionals to provide coordinated care for patients
• Maintain accurate and up-to-date patient recordsAdhere to all legal and ethical standards in accordance with state and federal regulations

Qualifications:

• Bachelor's or Master's degree in Physical Therapy from an accredited program
• Current state license to practice as a Physical Therapist
• Minimum of 2 years of experience in a clinical setting, preferably in a senior living or skilled nursing facility
• Strong knowledge and understanding of physical therapy principles and techniques
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
