Job summary

Lacamas Creek Post Acute in Camas, WA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. Full-time or PRN roles available, with a sign-on bonus of $5,000 paid over 12 months.

You will evaluate patients, develop plans of care, provide skilled PT services to improve strength, mobility, balance, and function, and collaborate with a dedicated interdisciplinary team to deliver high-quality patient outcomes.

Responsibilities

• Evaluate patients and develop care plans.
• Provide skilled PT services to improve strength, mobility, balance.

• Document evaluations, treatments, progress, and outcomes.
• Collaborate with PTAs, nursing, and interdisciplinary team.
• Supervise PTAs as appropriate.
• Create a positive rehabilitation experience for every patient.

Physical Therapist (PT) - SIGN ON BONUS!!! Lacamas Creek Post Acute Pay: Up to $60 per hour for full time Sign on bonus of $5,000 paid in quarterly installments over 12 months.

Lacamas Creek Post Acute is looking for a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team. We're looking for someone who loves helping patients regain strength, mobility, and independence while being part of a supportive and collaborative team. What You'll Do

• Evaluate patients and develop individualized plans of care based on their needs and goals
• Provide skilled physical therapy services to help patients improve strength, mobility, balance, and overall function
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients, families, and caregivers on exercises, mobility, and strategies to support continued progress
• Accurately document evaluations, treatments, progress, and outcomes
• Collaborate with Physical Therapist Assistants, nursing, and other members of the interdisciplinary care team
• Provide guidance and supervision to Physical Therapist Assistants as appropriate
• Help create a positive, encouraging rehabilitation experience for every patient
