Job summary

Remedy Home Health Care in Orange County is seeking a licensed Physical Therapist (PT) for a Per Diem/PRN Home Health role. You will deliver skilled therapy to seniors in patients’ homes, helping them regain independence with fall prevention, strength, and mobility training.

You will collaborate with nurses, case managers, and physicians, documenting visits in WellSky EMR and supporting a community-based care approach across Orange County.

Qualifications

• Licensed Physical Therapist (PT) in California required.
• Minimum 1 year of clinical experience; home health preferred.
• CPR certification (AHA) required.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.

Key Responsibilities:

• Conduct in‑home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settingsEducate patients and families on fall prevention and safe mobility techniques
• Document visits using WellSky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams

Qualifications

• Licensed Physical Therapist (PT) – California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance
