Location/Department: Rehab Center - Physical Therapy
PRN
Job Summary:
Provides physical therapy to inpatients and outpatients. Responsible for hands-on care and associated documentation. Performs operational tasks of the department and participates in development activities.

Duties/Responsibilities:
- Selects appropriate evaluation tools.
- Administers evaluations correctly and safely.
- Uses correct documentation tools for patient types.
- Demonstrates competency in assessment of inpatients and outpatients.
- Develops appropriate care plans for patients.
- Sets clear, measurable goals.
- Creates effective therapeutic treatment plans.
- Initiates and progresses treatment based on patient progress.
- Performs treatments safely for patients and staff.
- Manages time effectively in planning and applying care.
- Modifies treatment based on ongoing assessment.
- Maintains effective communication with healthcare team members.
- Performs timely documentation with appropriate terminology and format.
- Utilizes EHR and other hospital systems effectively.
- Enters charges accurately for treatments provided.
- Participates in patient care meetings as appropriate.
- Represents the PT department in interdepartmental meetings.
- Provides patient and family education routinely.
- Includes written instructions and home programs for patients and families.
- Provides in-services to team members or community as needed.
- Participates in departmental meetings and in-services.
- Engages in outside workshops and continuing education.
- Contributes to departmental improvements and new program development.
- Involved in new team member orientation.
- Provides supervision to PTAs, aides, students, or volunteers.
- Participates in peer and competency reviews.
- Familiar with federal and state regulations for PT services.
- Understands insurance regulations relevant to PT services.
- Adheres to hospital and PT policies and procedures.
Required Skills/Abilities:
- Strong customer service skills.
- Effective verbal and written communication skills.
- General physical therapy treatment skills.
Education and Experience:
- Bachelor’s degree in Physical Therapy.
- Licensed as a Physical Therapist in Indiana.
- No prior experience required.
Physical Requirements:
- Frequent walking, standing, reaching, talking, and use of hands.
- May need to lift up to 25 lbs; occasionally up to 50 lbs.
