Chestnut Health seeks a compassionate Nurse Practitioner to join its physiatry team in Illinois. You will collaborate with therapists and nursing staff to design and execute comprehensive rehabilitation plans, conduct assessments, and monitor patient progress to promote independence.

Qualifications

• Certified Nurse Practitioner (NP) license active in Illinois
• DEA license required upon start date
• Controlled Substance license required upon start date
• ANCC or AANP certification recognized
• Desire to work in Physiatry/PM&R
• Experience in orthopedics or physiatry a plus
• SNF/nursing experience a plus

Responsabilités

• Collaborate with therapists, nurses, and healthcare professionals to implement comprehensive rehabilitation plans.
• Perform patient assessments, histories, exams, and diagnostic evaluations to set rehab goals.
• Monitor progress and adjust treatment plans to optimize functional outcomes.
• Participate in multidisciplinary meetings, case conferences, and education to enhance skills.
• Document evaluations, treatment plans, progress, and discharge summaries in EMR.
• Educate patients and families about rehabilitation processes, outcomes, and post-discharge care
