Job summary

Chestnut Health is seeking a Nurse Practitioner to join our Physiatry team, focusing on rehabilitation and functional independence. Qualifications

• Active state license to practice in the employed state.
• Current Controlled Substance License required upon start date.
• Current DEA License required upon start date.
• ANCC or AANP certification required.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a nice-to-have.
• Physiatry experience is a nice-to-have.
• Skilled nursing experience is a nice-to-have.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for rehabilitation needs.
• Conduct patient assessments including histories, physicals, and diagnostics to set rehab goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehabilitation process, outcomes, and post-discharge care.
