Job summary
Chestnut Health is seeking a Nurse Practitioner to join our Physiatry/PM&R team in Illinois. New graduates are encouraged to apply for a role that emphasizes collaborative care, patient-centered outcomes, and comprehensive rehabilitation.
You will work with therapists, nurses, and physicians to develop and implement treatment plans, conduct assessments, monitor progress, and educate patients and families about recovery and post-discharge care.
Qualifications
- Eligible to work in the United States.
- Active state license preferred.
- Current DEA license preferred.
- ANCC or AANP certification preferred.
- Interest in Physiatry / PM&R a plus.
Responsibilities
- Collaborate with therapists, nurses, and physicians to design rehab treatment plans.
- Conduct patient assessments and evaluations to set rehabilitation goals.
- Monitor progress and adjust plans to improve function and independence.
- Participate in multidisciplinary meetings and ongoing education.
- Document evaluations, plans, progress notes, and discharge summaries in EMR.
- Educate patients and families about the rehab process and post-discharge care.
