Job summary

Chestnut Health in Pittsfield, Illinois is seeking a Nurse Practitioner to join our Physiatry team, delivering rehabilitation-focused care to help patients regain function.

You will collaborate with therapists and nurses, perform assessments, evaluate progress, and contribute to multidisciplinary treatment plans, with emphasis on education and independence.

Qualifications

• Eligible to work in the United States.
• Active license to practice in the state of employment preferred.
• Current Controlled Substance License by start date preferred.
• Current DEA License by start date preferred.
• ANCC or AANP certification preferred.
• Interest in working in Physiatry / Physical Medicine and Rehabilitation is a plus.
• Experience in orthopedics, physiatry, or skilled nursing is a plus.
• Certified Nurse Practitioner certification preferred.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
