Job summary

Chestnut Health Company seeks a Nurse Practitioner to join our Physiatry team, delivering patient-centered rehabilitation in collaboration with therapists and nurses. New graduates are highly encouraged to apply, with mentoring and an emphasis on improving function and independence.

You will conduct assessments, develop treatment plans, monitor progress, document in EMR, and educate patients and families about the rehabilitation process and post-discharge care in a supportive, team-based

Qualifications

Licensed to practice in the state of Illinois.
DEA license required upon start date.
ANCC or AANP certification required.
Interest in Physiatry/PM&R; orthopedic/PM&R experience a plus.
New graduates encouraged to apply.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
Conduct patient assessments, including medical histories and physical exams, to set rehabilitation goals.
Monitor progress and adjust plans to optimize function and independence.
Participate in multidisciplinary meetings, case conferences, and continuing education.
Maintain accurate EMR documentation of evaluations, plans, progress, and discharge summaries.
Educate patients and families about rehabilitation processes, outcomes, and post-discharge care.
