Job summary

Chestnut Health Company in Payson, AZ seeks a compassionate Nurse Practitioner to join our physiatry team serving residents in Skilled Nursing Facilities. This role emphasizes collaborative, patient-centered rehabilitation to restore function and independence and to improve quality of life for patients.

You will perform thorough assessments, partner with therapists and nursing staff to craft treatment plans, document progress in EMR, and educate patients and families.

Qualifications

• Active license to practice in the state of employment
• Current Controlled Substances license (required on start)
• Current DEA license (required on start)
• ANCC or AANP nurse practitioner certification
• Desire to work in Physiatry/PM&R

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs
• Conduct thorough patient assessments including histories and exams to determine rehabilitation goals
• Monitor patient progress and adjust treatment plans to optimize functional outcomes
• Participate in multidisciplinary team meetings and educational activities
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems
• Provide education and counseling to patients and families about rehabilitation and post-discharge care
